What to Bring to Your Appointment

Bring the following with you when you come to Advanced Cardiac Specialists for your appointment:

  • A list of your current medications and dosages. You may bring the bottles or containers with you to your first appointment.

  • Medical information and/or reports, radiology images, if requested by Advanced Cardiac Specialists.

  • If you received your new patient packet in the mail, please fill out all forms and be sure to bring them with you to your appointment, this will speed up the check in process.

  • The name, address and telephone number of the doctor who referred you, if applicable.

  • We will need your insurance card.  If you are covered by a secondary insurance bring both insurance cards including your Pharmacy card.

  • If you are covered by an HMO/PPO insurance that requires a written referrals to a specialist, be sure to bring the written referral from your Primary Care Physician.

  • A copy of your living will/advance directive for your doctor to review, if you have one. It will then be placed in your medical record.  If you have not completed a living will/advance directive we can supply the form to you be sure and ask the office staff.


 

Making an Appointment

Advanced Cardiac Specialist's scheduling department is staffed with caring and knowledgeable personnel who will help you make an appointment. +They will ask you pertinent questions about your health, may request insurance information, and will inform you about the services that are offered at our practice, all in an effort to best accommodate your health care needs.

  • To make an appointment, please contact our central scheduling department at 480-474-3555 or our toll free number 1-800-73PULSE (1-800-737-8573). Hours are 8 a.m. to 5 p.m. Monday through Friday.
  • Appointments are important to both the patient and the health care provider.
  • Appointments must be cancelled 24 hours in advance.
  • Appointments that have not been cancelled are considered a "no show". All "no show" appointments will be charged a $25.00 fee.

Pre-Surgical Clearance

At Advanced Cardiac Specialists, we frequently help our patients and those referred to us prepare for surgery.

Pre-operative clearance, includes and history and risk factor evaluation that will be used by the surgeon and anesthesiologist in preparing you for surgery. Patients that are over 50 years old and those that are younger with medical problems or on prescription medications will need clearance. The pre-surgical clearance routinely includes CXR, EKG and laboratory testing. Frequently, depending on the patient's medical and family history it may include Cardiovascular stress testing, management of anti-coagulation and coumadin , as well pulmonary testing.

The evaluation will be promptly sent to your surgeon to help in scheduling our upcoming surgical procedure. Our providers can do a cardiovascular screening or a general medical screening.

Please allow 2 weeks prior to your surgery to complete the screening so all of the information may be available to your surgeon.

New Patient Registration

A New Patient Representative is available to assist you from 8:00 a.m. to 4:30 p.m. Monday through Friday by calling 480-288-4089.

When planning your first visit to your new physician, if possible fill out our New Patient History Form. Fill out the forms and bring them with you at the time of your visit. Bring a list of medications and if there are questions bring all of your medications with you. You will also be asked for your insurance card and identification at the time of check-in.

New patient's plan to arrive 30 minutes prior to appointment time and established patients plan to arrive 15 minutes prior to appointment time.

Welcome to Medicare Physicals

This exam is a one-time exam to be completed within the first year of enrollment into Medicare Part B. The "Welcome to Medicare" physical exam is a great way to get up-to-date on important screenings and shots and to talk with your doctor about your family history and how to stay healthy.

What to expect during the exam:

  • Your doctor will record your medical history, check your blood pressure, vision and weight and height to measure your body mass index.
  • Your doctor will check to ensure you are up-to-date with preventivie screenings and services, such as cancer screening and shots.
  • Your doctor will give you advice to help you prevent disease, improve your health or stay well. You will receive a written plan similar to a checklist when you leave letting you know which screeings and other preventive services you should get.
  • Your doctor will talk to you about end-of-life planning, including advance directives. Advance directives are legal documents that allow you to put in writing what kind of health care you would want if you were too ill to speak for yourself.
  • Your medicare deductable will be waved but you will be responsible for 20% of the allowable.

What to bring to you Welcome to Medicare exam:

  • Medical records, including immunization records (if you are seeing the physician for the first time)
  • Family health history
  • A list of prescription drugs that you are currently taking, how often you take them, and why.

Remember! Once you enroll in Medicare Part B, it's important to schedule your "Welcome to Medicare" physical exam right away.
