Medical Records

The team in our medical records department is actually your team; they protect YOUR privacy and follow all the HIPAA guidelines.

Your medical records are invaluable to you as a patient as well as your group of physicians that provide your medical care. It is your responsibility, as a team, with your health care provider to keep this information accurate and up-to-date. Your personal  data is stored electronically and with this information ACS staff has set guidelines, which are based on research by the American Board of Internal Medicine and the American Heart Association to keep you healthy and prevent disease whenever possible. Many of you go to several specialists in addition to your providers at ACS; please try to help maintain an open communication for exchange of information between all of your providers.
 
When going to see a referral physician within ACS or outside of ACS, please be sure you have the information sent to your physician prior to the appointment. Our medical record team will not know of your outside appointments and the date of these appointments even if our referral department was responsible for setting up the authorization.

Requesting records:  Records to other physicians' offices will be sent over at no charge, please allow at least one week for this transfer of information.

If you need records sent to another person, attorney or family member, an official record release and authorization must be completed. All records are reviewed prior to going to disability or legal sources; this process can take up to 2 weeks to complete.

Any forms for disability and FMLA will be charged a flat rate of $25.00
