Innovations and Leading-edge Research in

occluder device is being used in FDA-monitored clinical trials
in select sites across the country. Advanced Cardiac Specialists
was the first research site for this device in Arizona and one
of six in the southwestern U.S. (This device is available only
as a research/investigational device and is not availed
routinely at other Cardiology clinics or hospitals.)

24/7 Hospitalist Services at Mesa General
Hospital
Patients may be referred to our Hospitalist
Service at Mesa General Hospital. They receive quality care during
their hospitalization and are referred back to their physician
when they are discharged from the hospital. All hospitalists are
Advanced Cardiac Specialists physicians and interact with multiple
sub-specialty physicians to provide "seamless"
in-hospital patient care.
Urology at Gilbert/Mesa and Globe locations
Office procedures include:
Cystoscopy
Prostate biopsy
Vasectomy
Transurethral microwave therapy in Benign
Prostatic Hyperplasia (enlarged prostate).

reliable means of assessing bone strength and predicting
risk of fracture
o The test is
used to monitor the effects of treatment.
Tilt Table Testing at Apache Junction,
Gilbert/Mesa, Goodyear, Phoenix and Prescott offices (supervised
by qualified Electrophysiologist)
o For the evaluation of syncope
in patients in whom the diagnosis of vasovagal or
neurocardiogenic syncope is entertained.
MTWA (Microvolt T-wave Alternans) Test,
for early detection of patients at risk for Sudden Cardiac
Death requiring automatic implanted defibrillator, at our
Gilbert/Mesa office
o "At-risk"
patients, who test MTWA-negative, will likely live
event-free for 12 to 24 months after their initial test
o Highest risk patients MTWA-positive
should be referred for Electrophysiology consult +/- ICD
implantation
o The test is FDA and
Medicare-approved for its ability to predict ventricular
tachyarrhythmias and Sudden Cardiac Death.

Medical Records
Please bring your
records with you when you return to Arizona. Please let us know
if your referring physician has changed so we can keep him/her
informed.
Please bring all
of the medications you are presently taking to your first seasonal
visit with your provider.

If you need to schedule
an appointment, please call our Phoenix East Valley Appointment Line

Please consider calling
us to schedule your appointment at least one month before your desired
appointment date. If you have not yet returned to Arizona, you
may call 1 (800) 73 PULSE (78573) and ask for Appointments.
We have multiple
offices. Your "home office" is where you see your
physician. For the most part, your chart will be in the office
where you last saw your physician. You should call that office
with prescription and medical questions, or there will be a delay to
courier your chart from another office. You may be connected to your
home office from any ACS telephone line.
For your reference,
physicians are seeing patients throughout the day so our medical staff
will help you or consult with them on your behalf. If you have a
life-threatening emergency, please call 911. For urgent, but not
life-threatening questions, please call our Nurse Line. Emergencies
are always worked into our schedule. However, out of respect for
our patients with appointments, we ask you to schedule visits for
non-urgent healthcare issues.

I nsurance
Authorizations & Referrals
If you have an HMO plan
that requires a referral or authorization to be seen by a specialist,
please have it either faxed to us at (480) 983-9743 or bring it with
you for your appointment.
Prescription Refills
While here for an
appointment, this is your time to review your medications to see if
you will be needing any prescription refills. Make sure to
discuss your prescription refill needs with your medical staff while
you are here. This will save you an extra phone call.
In between
appointments, if you have refills available on your current
prescription, please call your pharmacy at least five business days
before you are out. (Please allow an extra day as many
insurances are now requiring prior authorization for certain
medications.) If you are out of refills, or have questions about your
medications, please call our Pharmacy Line Monday through Friday from
8:00 a.m. to 5:00 p.m. (Please plan ahead. Please do not
call our on call physicians, after hours or weekends, for refills;
often they will not be able to adequately refill your medications.)
If you do no have an
appointment, but want to stop by to ask questions, we have a checklist
available at check-in to help you get your questions answered in a
timely manner. This checklist may be used for questions
regarding health issues, medications, refills, requests for copies of
labs, etc. The checklist will allow the nurse and provider to
review your medical record and promptly and properly answer your
questions.
Labs/Testing/X-rays
For your reference,
blood cannot be drawn without a physician’s order (from a current or
previous appointment). If you do not have a physician’s order
with you or on file, an appointment will need to be scheduled; or, you
may make this request on the pre-appointment checklist.
Lab, testing and/or
x-ray results are not available until the doctor reviews them.
Depending on the doctor’s schedule, it may take up to a week.
We will post the
results on our new private patient information line if your labs,
testing and/or x-ray are normal or if a minor adjustment in
medications is required. (A handout on how to access the private
patient information line using your touch tone telephone is available
at our office. Our staff can answer any questions you have
during your visit.) If your results require a follow-up
appointment, additional testing or a medication change, you will be
notified.
Coumadin Clinic
If
your Pro-time is not within range, we may need to schedule an
appointment with your physician. Otherwise, we will schedule a
Coumadin Clinic appointment for you. (You will need to see one of our
physicians once a year to participate in the Coumadin Clinic.)
Cardiac Rehab
If you are planning to
attend cardiac rehab, please call (480) 982-2800 and ask for Rehab at
extension 2037 to set up your readmittance. Please allow at least five
business days for staff to set up your new exercise time.
If your home telephone
screens for caller id, please note that our current phone system will
not allow us to identify our call. If you are expecting a call from
us, please consider other options since our call may not come through.
Flu Shots
If
you are a patient and would like a flu shot, we will take your name
and number to call you, when the vaccine is available, to schedule an
appointment for our Flu Shot Clinic.
Surgery
If you are having
surgery, especially elective surgery, please notify us at least a
month in advance so we have the time necessary to arrange for any
surgical clearances or testing that is required. (An appointment as
well as testing may be needed.)
ONCE YOU KNOW YOUR
SPRING TRAVEL PLANS . . .
Please make sure to
provide us with your local as well as your permanent address and phone
numbers.
When you know your
travel plans, please call at least six weeks in advance to schedule
your wrap-up appointment prior to your departure.
At least six weeks
before your departure, please request any medical records you may
need. There will be a nominal charge, after a minimum number of
pages, to cover costs. These medical records routinely only include
testing and lab and x-ray due in the winter season.
